Complete this form if you want to direct a staff member at Servant Solutions to change your selected investment options with respect to both EXISTING assets accumulated in your Retirement Plan account and FUTURE contributions to the Retirement Plan. You can also make an investment change yourself by logging into your Principal account (click “Personal Account Login” in the top right-hand corner above). If you would like to change only your existing asset investment election or only your future asset investment election, please contact our office for assistance.
